Replacement of Certificates

(a)Replacement of Certificates. A new Certificate of Registration, to replace any certificate lost, destroyed, or mutilated, may be issued, subject to the rules of the Board and upon payment of the prescribed fee, and such certificate shall be stamped or marked “duplicate”.

(b)Re-issuance of Certificates. The Board, for reasons it deems sufficient, may reissue a Certificate of Registration to any person whose certificate has been revoked, provided, that four

(4)or more members of the Board vote in favor of such re-issuance.
