Permit to Operate
View official PDF ↗(a)No elevator, dumbwaiter, escalator, moving walk or manlift shall be operated in any place of employment in Guam unless a permit for the operation thereof is issued by the Department, and unless such permit remains in effect.
(b)A permit to operate an elevator, dumbwaiter, escalator, moving walk or manlift shall be issued to the owner or lessee only after a commissioned inspector has found that such device has met all requirements of this Article.
(c)A permit to operate shall be valid for six
(6)months unless sooner revoked and may be renewed only upon completion of an inspection by a commissioned inspector.
(d)The permit to operate shall indicate the type of equipment for which it is issued and, in the case of elevators, shall state whether passenger or freight, and shall also state the rated load and speed for such elevator, dumbwaiter, escalator, moving walk or manlift. The permit to operate shall be posted conspicuously in the elevator and on or near the dumbwaiter, escalator, moving walk or manlift. COL11/29/2022 CH. 67 THE BUILDING CODE
(e)There shall be no charge for the processing and issuance of subsequent elevator operating permits issued as a result of a periodic inspection. However, the Department shall reserve the right to make a nominal charge not to exceed Two Dollars ($2.00), or as determined by the Director through the Administrative Adjudication process, for the processing and issuance of duplicate permits.
