Discharge Plan Requirement

(a)Any patient placed on assisted outpatient treatment must be given a treatment plan at the time of discharge from inpatient care and placement on an assisted outpatient treatment program for a period anticipated being greater than seventy-two

(72)hours.

(b)An outpatient treatment plan may include, but is not limited to,

(1)suggested medication;

(2)individual or group therapy;

(3)day or partial day programming activities; COL622016 CH. 82A ASSISTED OUTPATIENT TREATMENT

(4)services and training, including educational and vocational activities;

(5)residential supervision;

(6)intensive case management services; and
